3. I, in my behalf and/or in behalf of my minor child, hereby declare that I acknowledge that this event is a sporting event that may involve strenuous physical activity;
4. I, in my behalf and/or in behalf of my minor child, hereby declare that I herein waive any right to claim damages against the school, the organizers, the sponsors, and other staff and authorized representatives in the event of any untoward incident or accident;
5. I, in my behalf and/or in behalf of my minor child, hereby declare that I am physically fit to join this event as certified by a medical physician;
6. I, in my behalf and/or in behalf of my minor child, hereby declare that I hereby grant full permission to the organizers to use any photograph or any record of this event for any legitimate purpose;

Further, I hereby declare that the information I subscribed herein, particularly my name, age, citizenship, and postal address are true and correct and that I am willing to show proof of the same in case the organizers of the race request such information.
